The Chuck Rabaut EAP Florida Scholarship

The Chuck Rabaut EAP Florida Scholarship was established in 2003 at the direction of the Board of Directors.  It shall exist in perpetuity to provide assistance to members of EAP Florida and other potential EA practitioners.

It is the mission of EAP Florida to support the Employee Assistance EA  (EA) profession, including the certification of EA professionals (CEAP) and other activities in Florida that enhance the EA profession and the skills of those who work in it.

In seeking to fulfill this mission, and in recognition of one of our founders, Chuck Rabaut, and his tireless efforts in this regard, the scholarship shall fund the cost of the annual conference fees, one night's accommodations and an annual membership.  It shall be awarded annually, and the number of scholarships to be awarded each year is at the discretion of the Board.

Criteria for consideration of this scholarship shall include:

1. Stated need,

2. Letter of request, 

3. Letter of reference from a professional in the EA field.

EAP Florida Board members cannot apply for the scholarship, nor can they submit letters of reference for others' applications, during their term in office.

Application is to be made in writing to the Board, and submitted to the President prior to the first quarter meeting for that calendar year's award.  It is the responsibility of the applicant to ensure timely submission of the application.  The scholarship shall be awarded by a majority vote of the Board members present at the first quarter meeting.

27th Annual Florida School of Addictions Studies

Since 1983, the Florida School of Addictions Studies, Inc., has annually recognized an outstanding professional in the field of substance abuse treatment and/or prevention.  This effort is to promulgate a feeling that the student body is very important to the leaders, supporters, and sponsors of the School.  Nominations for this prestigious award are encouraged from any and all interested parties, including the FSAS Board or Directors and the FSAS Membership Committee; however, current/former members of the FSAS Board of Directors are not eligible for this award.

·         $500 cash award

ELIGIBILITY CRITERIA:

·         A person who has worked in the addictions field for a minimum of five (5) years

·         For those professionals working in the trenches who provides direct services to consumers and have consistently contributed to the field of substance abuse, having distinguished his/her self beyond the requirements of their position and demonstrated excellence in the field.

·         Available to attend the 2008 School July 20 through July 24, 2008

·         Current/former members of the FSAS Board of Directors are not eligible for this award

 

DOCUMENTATION REQUIRED:

·         Complete Nomination form and nominator’s letter setting forth justification for the nominee’s selection as Professional of the Year

·         Current Resume of the person being nominated for Professional of the Year

·         Two (2) letters of recommendation from nominee’s colleagues, peers, supervisors, clients, or informed others who can attest to nominee’s achievements.

NOTE:  Winner must furnish a 5-inch by 7-inch or larger photograph suitable for reproduction.  The recipient shall bear the expense of transportation, lodging, and any meals not furnished at the school.

The Florida School of Addictions Studies is pleased to announce that a limited number of scholarships are again available for students who demonstrate financial need and meet the criteria specified by the various agencies who donate scholarship funds. Scholarships include full tuition, breakfast & lunch Monday through Wednesday, and breakfast Thursday, as well as Tuesday evening’s dinner and special activities. Scholarships DO NOT INCLUDE the cost of dorm or hotel rooms for the four days.
